MEDICATION MANAGEMENT
At Olympia Hospice we offer medication management as part of our full suite of hospice services in Los Angeles and surrounding areas. We understand the importance of medication in maximizing the quality of life among individuals with life-limiting illnesses. One of our priorities is to ensure the proper timing, dosing, and tracking of medications to reduce the risk of adverse reactions, misuse, and dosing errors. During periodic visits, our care team develops a plan of care that involves tailored medicines to meet their unique needs.
We work closely with patients to determine the efficacy of their medication regimen.
Our team creates an individualized medication management plan that includes pain medications and any medications associated with a patient’s end-of-life diagnosis.
Adhere to medication schedules to prevent missed doses that can result in worsening pain and symptom attacks.
Keep track of medication administration provided by our hospice team and primary caregivers.
Educate patients and primary caregivers about the proper and safe administration of medications and treatments.
