At Olympia Hospice, we offer assisted living visits for terminally ill patients in assisted living or skilled nursing facilities in the Los Angles area and surrounding counties. While staff members in assisted living facilities specialize in long-term care, our team of hospice professionals is geared towards pain and symptom management for end-of-life care. Our assisted living visits include medical, emotional, personal care, spiritual, and grief support to hospice patients that reside in assisted living facilities and nursing homes

  • Consultations by our hospice physicians are provided by request or when needed.

  • Regular visits by home health aides to conduct personal care routines, including bathing, grooming, and feeding to complement the care provided by assisted living facility staff.

  • Emotional and spiritual support programs by our hospice team for hospice patients and their families.

  • Provide medical equipment, supplies, and medications to alleviate pain and symptoms and enhance comfort in their assisted living or nursing home setting.

  • Close coordination of a patient’s care plan, including medications across all medical providers, including resident physician, hospice care team, and assisted living facility staff.
